Wooster, OH Wooster, OH Full-time Full-time $40,000 - $50,000 a year $40,000 - $50,000 a year 5 days ago 5 days ago 5 days ago The Counseling Center of Wayne and Holmes Counties is actively seeking caring, professional Adult Therapeutic Behavioral Specialists / Case Managers to actively encourage and support the recovery of persons with severe and persistent mental illness; and assist psychiatrically impaired adults in achieving and maintaining successful community living.

Duties:
Coordinates and reviews patient evaluations to assure accurate needs of patients Develops with patient, a coordinated treatment plan to address patient needs including consumer empowerment, quality of life, access to care, housing, education, medical services, employment, and other areas identified by the patient Links patients to needed organizational and community-based services, advocates for services on their behalf, and assures access to necessary services is achieved Monitors psychiatric symptoms/condition, compliance with treatment plan, and coordinates service delivery with providers both within and outside of the agency Facilitates access to transportation to and from appointments Works with patient's family and/or significant others Completes necessary documentation Education and Credential Requirements A minimum of a bachelor's degree in social work or a related mental health area Licensed as an LSW, LISW, LPC, or LPCC preferred but not required.
